West Los Angeles, California / Private-pay home care

Keep more of the care revenue you already earn.

Overtime was eating the margin on an established book of care. Acensus helped the agency redesign shifts, correct underpriced cases and recover missed inquiries, without adding advertising.

Starting position

An $8.2M agency delivered 4,004 hours of care a week to 154 active clients. Monthly admissions averaged 7.4, below 8.1 discharges. Sixty-three caregivers worked more than 45 hours a week.

The constraint

More clients would not fix the cost of the existing schedule. Long shifts, overtime and 22 cases below the agency’s rate floor were reducing the gross profit left to fund growth.

What Acensus installed

Week 1 mapped each case to shift cost and overtime. Week 2 redesigned the 34 highest-cost cases with the agency’s existing roster. Intake and family-continuity scripts supported the changes. The agency reviewed underpriced cases, while Acensus installed missed-call text-back, after-hours routing and a weekly overtime and admissions Ledger.

The economics and what shaped the result

What the investment produced

$62,300Added gross profit realized during the 90 days
$466,107Annualized added gross profit · ending weekly pace × 52

The larger lever was the care already being delivered. The $1.54 hourly margin improvement on 4,004 opening weekly hours represents $320,640 in annualized gross profit. The additional 188 weekly hours at the ending margin add another $145,467. Those two components do not overlap.

Source: the client’s own billing and operating records. Investment includes Acensus fees and stated advertising or messaging costs. Gross profit is after direct care costs, before agency overhead; client staff time and other unlisted costs are excluded. Annualized figures are a forward operating pace, not cash collected over 12 months.

Reporting basis and how to read the figures

(4,004 opening hours × $1.54 margin improvement + 188 added hours × $14.88 ending margin) × 52. Investment: Acensus fees; no additional media spend.

Operating figures are client-reported endpoint comparisons. Historical client results are specific to the agency and engagement shown.

What else shaped the result

Six caregivers resigned during the changes. Nine families objected to a second caregiver; two reduced hours and one discharged. Three live-in cases retained their original shifts. Two clients left after rate corrections. The result includes those trade-offs.
